Determining eligibility

To determine eligibility for emergency assistance the following documents are required :

Social security cards for all household members.

Income documentation for at least the past three months for all household members who receive income or assistance.

Proof of residence showing a current Volusia County address (we cannot accept an address with only a post office box number).

A picture identification card, preferably a Florida driver's license.

Proof of a crisis in the home which has caused an interruption in the total family income, i.e., unemployment, disability, desertion, etc.

Proof of monthly expenses for the month (receipt of expenditures)

In addition the applicant must present the following documents:

Rental assistance - three day eviction notice.

Mortgage assistance - foreclosure notice or mortgage payment book.

Electric assistance - past due electric notice.

Prescription assistance - prescription from physician.

Assistance cannot be provided without these documents.
