Volusia County
Health Department
February 25, 2008
There are
changes to primary care health providers in west Volusia County. You
may need to transfer your medical records and choose a new doctor.
The Volusia County Health Department will end pediatric and adult
primary care health services at its Orange City clinic June 30 and
its Deltona clinic Sept. 30. As a result, you will need to choose a

secured by the Volusia County Health Department. Once you have
chosen a new doctor, you are asked to contact the health
department’s medical records office at (386) 822-6215 ext. 6123 so a
copy of your medical records can be forwarded to the new doctor.
Please choose a doctor by June 30 (DeLand patients) and Sept. 30
(Deltona patients).
