Make an appointment or apply online

Human Services accepts online applications for electric bill assistance at 9 a.m. on Mondays. Apply online here for electricity assistance beginning at that time. Online electricity assistance applications will be open until maximum capacity is reached.  

For rent, mortgage or water assistance, a phone appointment must be made by calling 386-239-7757. The appointment line opens at 8 a.m. every Monday, and appointments will be made until maximum capacity is reached. Electricity assistance may also be requested at this time via the phone appointment line.

For dental or prescription assistance, residents may apply in person at any office or schedule an appointment by calling 386-239-7757.

Submit documents

Persons who already have an upcoming phone appointment or have been asked to submit additional documents may upload verification documents through our secure portal.  Do not email your supporting documentation.  Please make sure eligibility documents are uploaded at least 2-days prior to your appointment.   An automatic email will be generated to you if the documents were uploaded successfully.  A person may also drop-off or mail supporting documents instead of uploading via the secure portal.  

All of the following documents must be provided in order to process an application:

Identity:

  • Social security cards for all persons residing in the household.
  • Driver's license, Florida ID, or U.S. Passport for all persons 18 years of age or older.
  • Birth certificate, DH-680, Driver's license, or U.S. Passport for all persons 17 years of age or younger.

Residency:

  • Renting- copy of full lease with landlord.
  • Homeowner- copy of either the property tax statement or mortgage statement.
  • If your household is receiving a Section 8 voucher or on public housing you must provide a HUD- 50059 form, which is the Utility Allowance Worksheet.  You must request this form from your housing authority.

Income:

  • Proof of all income the household has received within the previous 30 days which may include but not limited to wages, SSI/SSDI award letter (current letter), SNAP award letter (current letter), retirement/pension, and child support.
     

Utility statement:

  • Provide a copy of the most current home energy (electric and/or fuel) statement issued by your home energy (electric and/or fuel) provider.  The bill/account must be in a household member's name included on the application for assistance.

UPLOAD DOCUMENTS

For phone appointments and online applications, please make sure eligibility documents are uploaded at least 2-days prior to your scheduled appointment.  Persons who have successfully completed an Online LIHEAP Application, and need to submit eligibility documents may utilize this secure portal. An automatic email will be generated to you if the documents were uploaded successfully.  A person may also drop-off documentation to one of our Human Service locations and our staff will make copies and complete the secure upload process. Review program list of needed documents carefully as applications will not be reviewed and completed without complete list of necessary documentation for all members of the household.
