Registration checklist
__ Complete and sign form
__ Attach copy of birth certificate for all youth participants
__ Include driver’s license number and phone number on check
__ Sign the release of liability
__ Keep a copy of this form and take it with you to the first day of the program
__ Must be received before the registration deadline (allow five days for mail time)

• Registration is provided on a first-come, first-served basis.

• Registration form and payment must be received at least three working days prior to the first class session.

• Registration will not be accepted after the first class session.

• The registrar will assist in accommodating any person with a disability wishing to register for a program.

• Cancellations - activities that do not meet the minimum enrollment will be canceled.

• Refunds will be issued if a class or activity is cancelled or if this office receives a notice of cancellation no later than three working days prior to the start of the program.
