This form is provided for the convenience of those firms submitting a proposal in response to ANY Volusia County solicitation. This form gives the firm an opportunity to be certain that there are no unresolved code violations or past due debt PRIOR to submitting a proposal.

Complete the top portion of the Vendor Clean Hands Search Request form along with the Officers & Significant Stakeholders form and fax them to the five individuals listed below or to the Purchasing Agent referenced on the form. Do NOT return the Vendor Clean Hands Search Request form with your proposal, however, the Officers & Significant Stakeholders form must be notarized and returned with the proposal regardless of whether you choose to utilize the Vendor Clean Hands Search Request form or not.
